What is pelvic health physical therapy?

Pelvic floor physical therapy is specialized care focused on the pelvic floor muscles, connective tissue, and nervous system that support the bladder, bowel, uterus/prostate, and spine. These muscles play a role in continence, sexual function, core stability, breathing, and everyday movement. We approach pelvic health with a whole-body lens, recognizing that it can be influenced by factors ranging from jaw tension to running gait. 

Pelvic health PT can help with symptoms such as leaking, pain, pressure or heaviness, difficulty with bowel movements, pain with intimacy, or challenges returning to activity after pregnancy or surgery.

Who can benefit from pelvic health physical therapy?

The short answer, nearly everybody! Some common areas include: 

  • Pregnant or postpartum

  • Experiencing bladder or bowel leakage

  • Living with pelvic pain or pain with sex

  • Recovering from childbirth, surgery, or trauma

  • Noticing pelvic pressure or prolapse symptoms

  • Returning to exercise, work, or sport

  • Managing chronic pain or muscle tension

What does a pelvic health physical therapist do?

A pelvic health physical therapist is a licensed physical therapist with additional training in pelvic health. We assess how your pelvic floor functions as part of your whole body, including posture, breathing, strength, coordination, and movement patterns.

Treatment is individualized and may include:

  • Education and body awareness

  • Breathing and relaxation strategies

  • Gentle strengthening or coordination exercises

  • Manual therapy (only with your consent)

  • Movement retraining and strength training for daily activities or exercise

Will there be an internal exam?

An internal pelvic exam is never required.

If an internal exam is recommended, your therapist will:

  • Explain why it may be helpful

  • Describe exactly what it involves

  • Ask for your explicit consent

  • Respect your right to decline or stop at any time

Many people improve without internal exams. Your comfort and autonomy always come first.

I’ve experienced trauma — is pelvic health PT still an option?

Yes. Pelvic health physical therapy can be a supportive and empowering experience for people with a history of trauma.

Our care is trauma-informed, meaning:

  • You are always in control of what happens

  • Consent is ongoing and can be withdrawn at any time

  • We move at your pace

  • External-only treatment is always an option

  • You are never required to share personal details

What should I wear to my appointment?

Wear comfortable clothing that allows you to move easily, similar to what you’d wear to a regular PT visit or gentle workout. You’ll remain covered and draped appropriately at all times.

How long will I need pelvic health physical therapy?

The length of care varies based on your goals and symptoms. Some people benefit from just a few visits focused on education and guidance, while others require several weeks or even months of care. Progress and next steps are always discussed together.

Do I need a referral?

In Texas, you can begin physical therapy without a referral under direct access for a limited period. If ongoing care is recommended beyond that timeframe, we’ll help coordinate a referral with your provider.

Is pelvic health physical therapy only for postpartum patients?

No. While postpartum care is common, pelvic health PT supports people across the lifespan, including athletes, menopausal patients, people with chronic pain, and those who have never been pregnant.

What if I’m nervous or unsure?

That’s completely normal. You don’t need to know the “right” words or have a diagnosis to start. Questions, pauses, and uncertainty are welcome. Pelvic health physical therapy is a collaborative process, and your comfort matters.

What is my first appointment like? 

You'll be welcomed as soon as you arrive and you'll head into a private treatment room with your physical therapist. You'll be able to share your story, symptoms and goals without feeling rushed.

You'll get a thorough, head-to-toe evaluation.  You'll get treatment on the first day, and you'll leave with an 'Action Plan' for going forward for how to meet your goals.

We know coming to pelvic health therapy can be intimidating - you'll be taken care of throughout your time at Pelvic Wise and we’ll make sure you're comfortable throughout your treatment with us.


Do you accept insurance?

We are an out-of-network provider and do not bill insurance directly. However, we can provide a superbill for you to submit to your insurance company for possible reimbursement. Being out-of-network also allows us to offer care without insurance-imposed caps or time limitations.
